For Therapy:
$180 per 55-minute session or $270 for 85-minute session

For Bariatric Surgery Psychological Evaluation:
Rates vary depending on the requirements of your surgeon but generally range between
$350 and $450. 

Reduced Fee
I have a limited number of sliding scale appointment slots. Sliding scale appointment slots reduce to $120/55 minutes, depending on your income. Income verification may be required. Currently these slots are full.

Payment
Cash, check, PayPal, and credit card are accepted for payment. 

Insurance

I am a provider for Anthem Blue Cross. 

However, depending on the census of my caseload, I may or may not be accepting particular insurances at any given time. In addition, certain health insurance policies may provide some coverage for “out of network” mental health treatment, others may not. Some may have a deductible that must be met before the insurance benefit begins, others may not.

My services are provided and charged to you, not to your insurance company, so you are responsible for payment for each session at the time it is held. Insurance company determinations and reimbursement occurs after you have paid for clinical services. Please note that many insurance companies that provide reimbursement will only reimburse you for a portion of the fee you have paid for services.

At the outset of therapy, it is important for you to find out exactly what mental health services your insurance company covers by asking them:

• 1. Whether or not they pay for “out of network” providers

• 2. How much they pay

In turn, I will complete statement of services which will include the information you need to receive these benefits.
